If you are being treated for a condition that may require the use of a controlled substance, your healthcare provider may enroll you in the Perspectives in Care. Controlled substance therapy comes with certain risks that can negatively impact your treatment and screening for those risks can be challenging. The purpose of this program is to assist the doctor with identifying risk in order to find the very best treatment for you.
Here’s what you need to do next…
Visit the enrollment website (Perspectivesincare.com)
Click/tap the Patient Login button.
Log in with our Clinic Pin 153642 and complete the Care Continuity evaluations on each page.
When you’re done, click/tap the Submit button to complete your evaluation.
This evaluation must be completed before your next doctor’s visit or you may be required to reschedule your appointment.
